Recipient Rights Services
It is the philosophy of AuSable Valley CMH to protect and promote the dignity and respect to which each recipient of services is entitled.  The Michigan Mental Health Code protects these rights.

Some of your rights include:

The right to be free from abuse and neglect
The right to confidentiality
The right to be treated with dignity and respect
The right to treatment suited to condition

A copy of the booklet,"Your Rights ~ When Receiving Mental Health Services in Michigan" is provided to each consumer of AuSable Valley CMH
If you believe that one of your rights have been violated, you (or someone on your behalf) may make a Recipient Rights Complaints.  Our Recipient Rights Officer will review the complaint and may conduct an investigation.

The form is now available in Microsoft Word and Adobe Acrobat (PDF).  You will need Adobe Acrobat Reader to be able to view and print this form.  The PDF format can only be printed and then filled out by hand.  The Microsoft Word format can be saved to a hard drive or diskette, filled out on your computer, and then printed out and mailed.
